**T128 Approvals source flow column（2026-05-21 台北）**：
- 觸發：T125-T127 已把 source flow 帶到 Status Chain、Runs list、Work Items，但人工審批入口 `/awooop/approvals` 仍缺獨立掃描欄；operator 在 manual gate 需要先知道 source evidence 停在 Provider、Evidence、Applied 或 Verified。
- 修正：`apps/web/src/app/[locale]/awooop/approvals/page.tsx` 新增 `來源流程 / Source Flow` 欄位，直接讀取既有 `approval.awooop_status_chain.source_refs.correlation`。分類為 Verified、Applied、Evidence found、Provider received、Waiting；欄位顯示 `providers/direct/candidate/applied`。不新增 API、不新增 mock data；文案使用既有 `awooop.listEvidence.sourceFlow` i18n key；UI 使用 lucide icons。
